INITIAL ASSESSMENT SESSION

$150 The initial appointment involves reviewing intake paperwork, consent for treatment, completing a comprehensive bio-psycho-social assessment covering current issues and your past experiences. We will discuss recommendations for treatment, identify goals, treatment plan and answer any questions you have. 

INDIVIDUAL THERAPY SESSIONS

$150 - Client and therapist will collaborate on treatment goals and objectives to achieve desired outcomes. We will track progress and adjust treatment as needed to help improve well being and reach your goals.

GROUP COUNSELING

Group pricing will be determined  according to the number of sessions.

INSURANCE INFORMATION

In Network: Cigna, Aetna, United Health Care, Optum, & Oxford

I am also an Out of Network Provider.  If you have health insurance with Out of Network coverage, a percentage of the fee you pay for therapy will get reimbursed directly to you by your provider. You may have more coverage for Out of Network benefits than you realize.  

You will be responsible to pay in full prior to each session. 

You may also want to contact your insurance company prior to beginning therapy to ask the following questions:

• Do I have out-of-network benefits? If so, how much does my policy cover if I see an out-of-network provider?

• Does my current policy have a deductible? What is the amount, how much has been met this year, and when does my policy renew?

• Is there a limit to how many therapy sessions I can attend in a year? If so, how many?

• Before meeting with an out-of-network provider, do I need to get prior authorization or do you need any information on the therapist?

• How do I submit a claim for my individual therapy sessions? What specific information do I need?

SLIDING SCALE

 I have a few appointments available at a reduced rate based on need.

If you would like to be considered for this option please inquire about this during your initial consultation call. 

CANCELLATION POLICY

There is a 24 hour notice cancellation policy. If you do not provide 24 hours notice or if you provide less than 24 hours notice, you will be responsible for paying for the session.

Cancellation/rescheduling can be made by phone call, text, or e-mail. I will respond to confirm I received your message.
